Playtest Feedback Form

What this printable helps you do

Collect specific feedback on a game prototype. Start with the smallest playable or usable version. Define the participants, materials and available time before adding complexity. A brief trial often reveals an unclear rule faster than a polished page does. Ask for concrete examples of what happened, record the version tested and keep proposed changes separate from observations made during the activity.

Playtest Feedback Form printable with labeled areas: Prototype version, Moment of confusion, Interesting choice, One change to test

What is on the page

Four large writing panels separate related parts of the task. Each panel has its own heading and ruled space. Use short notes, labeled sketches or a mixture of both, keeping each detail beside the heading it helps explain. The four headings are Prototype version, Moment of confusion, Interesting choice, One change to test. Add a subject identifier and date at the top so the page can be understood when you return to it.

How to use it

Ask players to describe a moment they experienced rather than giving only an overall score. Start with one example rather than filling the whole page at once. Use ordinary language you will understand later and add a reference where a note depends on another source.

A practical example

A player who could not tell when a turn ended provides a clearer revision cue than confusing game. This illustrates how to begin; the downloadable page is blank so you can use your own subject, materials and observations.

Review before you reuse it

Keep player comments separate from your interpretation and test one major change at a time. Read the finished page from the perspective of someone who was not present. Clarify an abbreviation, mark an unanswered question and identify the next small action if one is needed.
