Choose one outcome for this page
This goal action plan printable is built around a single goal. Six sections move from the outcome and its purpose to evidence of progress, actions, resources, obstacles and a first step. Keeping one goal on each page makes it easier to see whether the proposed actions actually support the result you want.

Describe progress in observable terms
Write something you can notice or check, such as completing a draft or practicing a skill on chosen days. Separate actions within your control from results that depend on other people. The resources section can include time, materials or support you may need to arrange. Avoid filling every box simply to make the page look complete; uncertain details can remain questions.
Use the review date to revise the plan
Identify one small next action and a time to review what happened. In the obstacle section, write a realistic adjustment rather than a promise that nothing will interfere. At the review, compare your observations with the progress description and change the plan if needed. This is a planning prompt, not a guarantee of an outcome or a substitute for specialist advice.
